1. From your Cocoon task list, find the task that says “Download and complete your private disability claim” and then click “View forms”:

2. Verify your personal information and click “I’m ready to file my claim”:

3. Click “Download”:

4. Once you and your physician fill out the forms, return to your Cocoon task list and click on “Upload your completed private disability claim”, and then click on “Upload forms”

6. Our team will review the forms and submit the claim. You’ll receive an email confirming submission to the insurance carrier, the below screenshot shows an email confirmation of a claim submission to MetLife:
