Instructions for submitting your claims forms for private disability insurance to Kaiser for medical certification

Kaiser requires all requests for private disability claims needing medical certifications to be submitted to their members' local Release of Medical Information Department. However, this cannot be done online through members’ KP Portals.

Following are some lightweight instructions on how to get this completed:

  1. Locate the email address for your local Release of Medical Information office. Cocoon has provided you with links to these departments organized by region and located at the bottom of each of the pages linked below:

  2. Send an email titled with your name and your Kaiser medical record number along with the private disability claim forms Cocoon has provided you with attached.

    Here is a brief example of what that email might say:

    "I am submitting my private short-term disability claim forms for medical certification. Can you please certify my requested time via the cover letter attached and return them to me? Thank you so much!"

  3. Kaiser has their own template for medical certifications they will provide to you directly via email similar to the following:

  4. You will provide the completed certification directly to Cocoon by uploading it to your document center for review and final submission.
