In order to get your EDD claim medically certified by Kaiser, they require that you send the claim receipt number to their Medical Information Release Office. You can do this online through the KP portal.
Go to kp.org and sign in.
Scroll to the bottom of the page where it says "Member Support" and click "Medical Information Request"
3. Click on "State Disability Insurance certification
4. Click "Request SDI medical certification"
5. Answer the following questions:
Work Status Report refers to a referral to take time off work for disability, should be "Yes"
Form Receipt number is your EDD claim receipt number that starts with "R," followed by a series of numbers, should be "Yes"
If this is a disability extension then the claim number should start with "DI-" followed by a series of numbers
This is an initial claim if this is the first time you're requesting that your provider certified this claim. This is not an initial claim if you are requesting that your provider certify a date extension
