What to expect for your medical leave

Trigger: The day the leave plan has been submitted.

Hi [User],

Thank you for planning your medical leave in Cocoon! Taking time to focus on your health is incredibly important, but we know that things can feel uncertain when taking leave. We’re here to answer common questions and provide peace-of-mind ahead of your leave.

Leave forms

Once your medical leave plan is submitted, there are certain leave forms that need to be completed by your medical provider so that your leave can be approved for job protection and/or wage replacement. You’ll be directed to download these forms when you submit your leave.

Filing claims

You may be eligible for wage replacement benefits from state programs and/or your private disability insurance plan. After you submit your leave, Cocoon will guide you through the process to apply for these benefits.

Your insurance carrier may reach out to you for more information before they approve payments. These questions help them confirm that you’re unable to work. Some of these questions may feel invasive, but you can direct them to your medical provider if you don’t feel equipped to answer. Please reach out to us if you get any notices from insurance that you need help answering.

Leave pay

You should already be able to see where pay is coming from in your Cocoon leave plan.

If you have employer pay: you will continue to get a paycheck on your regular payroll schedule. However, if you have other pay sources, then your paycheck will be smaller to account for these other benefits.

If you have state and private insurance pay: your leave includes claims and in turn, benefit payments. Once you start to receive your benefit payments, save your pay stubs in case we need to verify that you’ve been paid correctly. Note that disability insurance includes a waiting period which means the first 7-14 days of your leave are unpaid by disability benefits.

Next steps

Now that your leave is drafted, the next step before you start your leave is submitting your leave plan, which will notify your manager and people team. In addition, you should work with your manager to start preparing your Out-of-Office plans.

What to expect for your upcoming leave

Thank you for submitting your leave in Cocoon. Taking time to focus on your or a loved one’s health is incredibly important, but we know that taking leave can come with a lot of uncertainty.

Here’s what you need to do.

Complete and return your leave forms

We need these forms to verify your eligibility. Download your forms from Cocoon. A medical provider will need to complete certain sections. When that’s complete, upload the completed forms back to Cocoon by 6/20/2025.

Reporting absences

Given you’re on an intermittent leave, for accurate leave reporting, you are required to report your absences in Cocoon.

Filing your claims

You are eligible to receive payments during your leave. To get your estimated full pay, you’ll need to file the following:

  • [Short-term disability insurance carrier]

Cocoon will guide you through how to apply, but we recommend familiarizing yourself with the process now. You may not get your full pay while claims are pending.

Getting paid

Getting paid on leave can be more complicated than your standard paycheck. Cocoon dashboard to see where your pay will come from throughout your leave.

Important things to know

  • Frequent absence reporting will minimize the number of paycheck adjustments.

  • If you’re eligible for money from the state or from private insurance pay, you will need to file claims to receive these payments. Don’t worry — we’ll remind you about this when it comes time.

  • Once you start to receive your benefit payments, save your pay stubs in case we need to verify that you’ve been paid correctly.

  • Most state and private disability programs include a waiting period. This means the first 7-14 days of your leave will not include any pay from disability benefits.

The Department of Labor requires employers to provide certain notices to employees who are taking a leave. You’ll find these attached. No action is needed on your end.

In some cases, you may not be eligible for leave through federal and state laws. However, you may still be able to take leave through your employer’s policy or follow the instructions in your dashboard to request a leave accommodation. You can learn more about your employer’s policy and these notices.

[Reminder] Report your medical leave absences

Please regularly update Cocoon with the absences you take.

Regularly keeping Cocoon up-to-date minimizes

back-and-forth between you and your employer, the number of payroll adjustments, if your leave is covered by your employer.

This email is sent out to the employee every 2 weeks to remind them to report their leave of absence hours.


Communicate about your return to work

Hi [User],

Your leave is set to end on [date].

If your return to work date changes, please update your leave dates so we can notify [employer]. Keeping Cocoon updated on your anticipated return allows [employer] to support you and plan for your smooth transition back. If your return to work date has not changed, there is no action needed.
