In this article, you’ll find a list of all the emails sent to employees and managers during a Medical Leave. In the "Nuanced" section, there are emails that may be sent in specific situations, with each email detailing the reason it’s being sent to the employee or manager.

Employee Path
These are the standard emails sent to employees before, during, and after their Medical Leave. Cocoon uses these emails to provide essential leave information and timely reminders, helping ensure a smooth experience throughout the leave process.

What to expect for your medical leave


Hi [User],

Thank you for planning your medical leave in Cocoon! Taking time to focus on your health is incredibly important, but we know that things can feel uncertain when taking leave. We’re here to answer common questions and provide peace-of-mind ahead of your leave.

Leave forms

Once your medical leave plan is submitted, there are certain leave forms that need to be completed by your medical provider so that your leave can be approved for job protection and/or wage replacement. You’ll be directed to download these forms when you submit your leave.

Filing claims

You may be eligible for wage replacement benefits from state programs and/or your private disability insurance plan. After you submit your leave, Cocoon will guide you through the process to apply for these benefits.

Your insurance carrier may reach out to you for more information before they approve payments. These questions help them confirm that you’re unable to work. Some of these questions may feel invasive, but you can direct them to your medical provider if you don’t feel equipped to answer. Please reach out to us if you get any notices from insurance that you need help answering.

Leave pay

You should already be able to see where pay is coming from in your Cocoon leave plan.

If you have employer pay: you will continue to get a paycheck on your regular payroll schedule. However, if you have other pay sources, then your paycheck will be smaller to account for these other benefits.

If you have state and private insurance pay: your leave includes claims and in turn, benefit payments. Once you start to receive your benefit payments, save your pay stubs in case we need to verify that you’ve been paid correctly. Note that disability insurance includes a waiting period which means the first 7-14 days of your leave are unpaid by disability benefits.

Next steps

Now that your leave is drafted, the next step before you start your leave is submitting your leave plan, which will notify your manager and people team. In addition, you should work with your manager to start preparing your Out-of-Office plans.

What to expect for your upcoming leave


Hi [User],

Thank you for submitting your leave in Cocoon. Taking time to focus on your or a loved one’s health is incredibly important, but we know that taking leave can come with a lot of uncertainty. Here’s what you need you do.

Complete and return your leave forms

We need these forms to verify your eligibility. Download your forms from Cocoon. A medical provider will need to complete certain sections. When that’s complete, upload the completed forms back to Cocoon by 4/10/2024.

Filing your claims

You are eligible to receive payments during your leave. To get your estimated full pay, you’ll need to file the following:

  • State - Short Term Disability claim

  • Private - Short Term Disability claim

Cocoon will guide you through how to apply, but we recommend familiarizing yourself with the process now. You may not get your full pay while claims are pending.

Getting paid

Getting paid on leave can be more complicated than your standard paycheck. Cocoon dashboard to see where your pay will come from throughout your leave.


Important things to know

If you’re receiving pay from your employer during leave, you will continue to get a paycheck on your regular schedule. The paycheck amount may be smaller if state programs or private insurance contribute to your total leave pay.

If you’re eligible for money from the state or from private insurance pay, you will need to file claims to receive these payments. Don’t worry — we’ll remind you about this when it comes time.

Once you start to receive your benefit payments, save your pay stubs in case we need to verify that you’ve been paid correctly.

Most state and private disability programs include a waiting period. This means the first 7-14 days of your leave will not include any pay from disability benefits.

Important information as you start your leave

If you started your leave before this date or have not started your leave yet, please update your plan in Cocoon. Incorrect dates can lead to issues with your leave or pay.

Please read this email carefully as it will help you prepare for important actions in the weeks to come!

File claims

Remember that you need to file the following to get your full pay:

  • State Short Term Disability

  • Private Disability Short Term Disability

We’ll walk you through the process of submitting each one. Visit your Cocoon dashboard to get started.

Verify your payments

Once your claims are filed and you begin to receive payments, remember to take a photo or save the check stubs you receive. We need this information to help [employer] calculate your paychecks while on leave.

Leave notice

Hi [User],

Thank you for submitting your leave through Cocoon.

The Department of Labor requires employers to provide certain notices to employees who are taking a leave. You’ll find these attached. No action is needed on your end.

Additionally, here are the benefits and protections offered by your state:

State Claim

In some cases, you may not be eligible for leave through federal and state laws.

However, you may still be able to take leave through your employer’s policy or follow the instructions in your dashboard to request a leave accommodation. You can learn more about your employer’s policy and these notices.

Here's an example of 'Leave Notice'

[Urgent] Remember to submit your leave forms


Hi [User],

You requested a medical leave from [employer]. Your request must be reviewed and approved in order for you to receive company benefits provided by [employer's] policy and job protection from leave laws like the Family and Medical Leave Act.


To apply for approval, [employer] requires that you submit completed medical leave forms.

Your time frame of 15 days for submitting the required forms ended on [date]. [employer] will provide you a one-time 5-day grace period to return the medical leave forms. If you do not submit them by [date], your leave will be denied, which could impact the leave benefits your employer offers. You can find detailed instructions for your leave forms (and re-download them if needed) in your Cocoon dashboard.

Your medical leave has been approved


Hi [User],

Your request to go on medical leave has been approved. If applicable, you’ll receive company benefits provided by [employer's] policy and/or job protection from leave laws like the Family and Medical Leave Act.

Next, be sure to file the claims you’re eligible for so that you can receive your full pay during leave. You can view your eligible claims in your Cocoon dashboard. Towards the end of your leave, you'll need to turn in your fitness for duty certification to return to work.

If your plans have changed, please update your plan accordingly so we can notify them of any changes. If your return to work date has not changed, there is no action needed.

Verify that you received full pay during your leave

Welcome back from your leave! Let’s make sure that you’ve received your full leave pay. First, confirm that you’ve received your last payment. Then, use Cocoon’s tool below to provide details about your payments from [State] and [Private Insurance].

We’ll help make adjustments if your actual pay differed from what was expected.

Manager Path
These are standard emails sent to managers once their employee has submitted a Medical Leave. These emails provide managers with updates to keep them informed about the status of their employee’s leave.

[User] has submitted a medical leave plan in Cocoon starting on [date].

Nuanced Emails

These emails are tailored to each employee's leave and specific situation. Employees will receive these emails based on their leave status and entitlements. Managers will receive these emails if their employee's leave has been denied or updated.

An employee will receive this email if they have not submitted their state claim within 14 days of deadline

Hi [User],


This is a reminder that you have 14 days left to submit your State STD claim.
You’ll need to file this claim to receive full pay during your leave.

Filing after the deadline can result in a delay or denial.

An employee will receive this email if they have not not submitted their private disability claim within 2 weeks of their deadline

Hi [User],

Cocoon has submitted your Private Insurance claim.

  • It’s normal for claims to take up to 3 weeks to be decisioned. Look out for any communication from [private Insurance] in case they need additional information from you. You can check on the status of your claims following the instructions here.

  • Once your claims are filed and you begin to receive payments from [private Insurance], take a photo or save the check stubs you receive from [private Insurance]. You can use our verification tool to make sure what you’ve received matches our pay estimates.

An employee will receive this email if their leave was denied due to a missing SHC Form

Hi [User],

To be approved for your requested medical leave, which is covered by [employer’s] leave policy and/or leave laws like the federal Family and Medical Leave Act, [employer] required that you submit completed medical leave forms.

You were given 15 days and an extension to return the forms. Because the required forms have not been submitted or remain incomplete despite requests for additional information, your leave has been denied. We’ve attached a notice confirming this decision.

Occasionally, it is not possible for employees to return the forms within this timeframe, despite their diligent, good faith efforts. If you believe that is the case for you, please contact your employer for next steps. You could also request leave as a reasonable accommodation under the federal Americans with Disabilities Act (request form here). Learn more about your options.

A manager will receive this email if their employee’s medical leave has been denied

Hi [Employer] team,

To be approved for their requested medical leave, which is covered by [employer’s] leave policy and/or leave laws like the federal Family and Medical Leave Act, employees are required to submit a completed Serious Health Condition (SHC) form.

Employees are given 15 days and then a 5 day extension, if needed, to return the forms. Because the required forms have not been submitted or remain incomplete despite requests for additional information, Sophia’s leave has been denied. [Employee] has been informed and received a notice confirming this decision today.

Occasionally, it is not possible for employees to return the forms within this timeframe, despite their diligent, good faith efforts. If Figma would like to support this leave request, you may:

  • Provide an additional two week extension for the employee to return their completed SHC form. To do so, please reach out to [email protected] to confirm the approved extension and applicable employee. Waive the requirement for the employee to return the serious health condition form and approve the leave.

  • Waive the requirement for the employee to return the SHC form and approve the leave. To do so, please reach out to [email protected] stating your approval and providing the approved leave dates.
