Certification for Continued Benefits - CA EDD

What is it?

Oftentimes, the EDD will issue a Claim for Continued Benefits which effectively pauses your disability payments until completed. Most commonly, the EDD will request a Claim for Continued Benefits if your disability is extended beyond the initial medical certification or if the time recommended by your provider extends beyond the standard recovery time for your specific diagnosis. However, the EDD will sometimes send this request out randomly, so it is helpful to keep an eye out for any messages in your SDI portal inbox, physical notices in the mail, or emails from the EDD’s end indicating action required.

When does this need to be completed?

The EDD requires this form to be completed within 20 days of it's publish date. As shown below, you will see a due date listed in your EDD inbox. The Certification for Continued Benefits needs to be completed by that due date. If not submitted by the due date, you may see a denial for the remaining benefit dates.

Steps to complete the certification and explain type of pay

  1. When reviewing your portal inbox you will see a message like the one pictured above

2. Once opened, you will need to click the link to get into the form to complete it

3. You can reference the pay tracker and pay period details on your Cocoon Dashboard for the calculations you'll need to complete this submission

Please note that the EDD will not accept this form until the Certification Period End date shown in Section 2 - Form Information has passed. The best day to submit is on the due date. If the due date has passed, submit this form as soon as possible. You may receive multiple of these requests for certification throughout your time out on leave.

Section 3 - Disability Status

While on leave you will want select that you were unable to do your regular work because of your disability throughout the certification period this request was received for

Section 4 - Money, Wages, or Workers’ Compensation

If you are receiving pay while on leave you will want to select “Yes”

Section 5A - Pay Information

  • Provide employer legal name for “Paid by

  • Provide “Paid from” and “through” dates

  • Provide “gross dollar amount received

Section 5A - Pay Information

  • From the options available you will want to select “Other” as payment type for top-up pay and then type “LSDI - Integrated Wages” in the box provided

Section 5B - Payment Information

This screen is reflecting the information you entered from Section 5A and allowing the option to add additional employer / wages from another source

Section 6 - Certification

Provide the applicable Cocoon Pay Program Explainer statement for online submission (due to character count limitation) in the “Additional Information” box to explain the type of pay being received while on leave.
