State-by-state breakdown of what we handle for employers, next steps for state programs, and tasks outside of Cocoon.

Here is a list of common requests we receive from states on behalf of employers. Often, states may contact your team directly for information that Cocoon might not have visibility into. We recommend working directly with the state for these requests. However, please feel free to loop us in to see if we can help partner in any way.

States Where We Handle Action Items on Behalf of Employers

Employees should use Cocoon’s address as the employer's address when submitting claims, as instructed by us, to ensure we receive the DE2503/DE2503F forms. DE2503 forms are completed online with employer account access, while DE2503F forms are filled out by hand and mailed to the EDD office. If we're unable to provision an account with the EDD, please forward any DE2503 forms directly to [email protected] for our team to complete.

If we have not yet set up an account with California, please follow these instructions to learn how to provide us with the necessary information.

Once an employee submits a claim to the state and notifies Cocoon through their Cocoon portal, we will then be prompted to complete and send the Employer Verification form to Aflac.

Cocoon does not supply the TDI-45 (HI’s claim form); employees need to obtain it from HI or their employer. After receiving the form, they should send it to Cocoon to complete the employer section. We will then return the completed TDI-45 to the employee for direct submission to HI.

We create a leave administrator account within the MAPFML portal to approve leaves on behalf of the employer. If we are unable to set up an account and you already have an account with MA PFML, you will receive notifications directly to approve the claim on behalf of the employee.

Cocoon will automatically populate the employer portion of the SDI and PFL claims, including our contact information to help ensure carriers reach out to us for any follow-up questions from the statutory carrier. The level of involvement from carriers can vary; for instance, Guardian often requests a time tracking sheet, which Cocoon will complete. Whenever possible, we'll complete the required documentation and will reach out to your team if additional information is needed to complete.

States Where Employers Handle Action Items Directly

Cocoon doesn't handle employer actions (e.g., verifications, ER forms). Currently, we do not offer TPA services for CO FAMLI, which involves assisting with wage reports and premiums, and is currently out of our scope.

Employers are not required to complete an “employer portion” of the application.

No action is required from the employer through Cocoon. The employee completes the claim online and provides RI with the necessary information.

Please note that an employee’s claim can occasionally be blocked or denied due to wage reporting issues. To prevent any potential claim issues due to wage reporting, please ensure that all required wages are reported accurately and on time. If any wage-related issues arise, the employee should be encouraged to contact your team directly so it can be resolved in a timely manner.

Cocoon can verify the claim if you provide us with the employee’s name and the unique identifier found on the claim form after the employee submits their claim. Alternatively, this can also be actioned by the employer.


If a state is not listed here, it may indicate that they do not have a program, require employer forms, or prefer to work directly with employers.
