Cocoon requires authorized access to assist with specific aspects of the claims process

During onboarding, you will be asked to provide Cocoon with information to represent your company in certain states where Paid Family Leave programs exist, Cocoon requires the following information to be established as an employer representative.

This allows us to manage claims on your behalf. If you have multiple entities, we may need additional information to create a state account for each applicable entity. State account setup is only required if you have employees working in those states.

If we can’t gain access to an account, it may result in notices being sent to you as the employer directly. This can add significant processing time to claims and may impact the decision processes at the state level, which can result in a delay or risk to benefits payments for your employees.

It’s not uncommon for state providers to occasionally restrict access and require re-verification. Cocoon may reach out when this happens to help reinstate the appropriate access.

State Accounts

California (CA EDD)

Gaining access to your employer CA EDD portal requires your most recent DE9 form, which provides us with:

  • Your employer account number (EAN)

  • The zip code you filed with the EDD

  • Total subject wages from the most recent quarter

The employer CA EDD portal allows us to complete Employer Notices DE 2503/F on your behalf.

If we're unable to create a portal account for any reason, please feel free to send these forms to our support team at [email protected] for completion.

New York State (Private Disability Carrier)

New York State's Paid Family Leave program is administered through a state-approved private carrier –Metlife, Guardian, Prudential, etc.-- for both the Disability and Family Leave components.

During your self-guided onboarding, you’ll be prompted to provide the name of your statutory carrier and policy number within the task list. This information will be used to automatically populate on the NY claim forms that employees will need to complete.

Massachusetts

Gaining access to your MassTaxConnect account requires your FEIN and Paid Family Leave Contributions amount from the most recent quarter. Employers must have an account registered with the Massachusetts Tax Board before Cocoon can be added as an authorized representative.

Access to your MassTaxConnect employer account allows Cocoon to approve claims on your behalf.

Hawaii (Private Disability Carrier)

Similar to New York State, Hawaii’s disability program is administered through a state-approved private carrier.

During your self-guided onboarding, you’ll be prompted to provide the name of your statutory carrier and policy number within the task list.
