How to complete the Certification for Continued Benefits (DE 2500A) form

What is it?

The EDD may issue a Claim for Continued Benefits, which temporarily pauses your disability payments until the claim is completed. Additionally, the EDD may send this request at random intervals, so it is important to monitor your SDI portal inbox, check for physical notices in the mail, and review any emails from the EDD that indicate action is required.

When Does This Need to Be Completed?


The EDD requires the completion of this form within 20 days of its publication date. As indicated in your EDD inbox, a due date will be provided for your reference. It is essential to complete the Certification for Continued Benefits by this due date. Failure to submit the form on time may result in a denial of benefits for the remaining eligible dates.

Steps to Complete the Certification and Explanation of Pay Types

  1. When reviewing your portal inbox you will find a message similar to the one illustrated above.

2. Once opened, you will need to click the provided link to access and complete the form.

3. You can refer to the pay tracker and pay period details on your Cocoon Dashboard for the necessary calculations to complete this submission.

Please be aware that the EDD will not accept this form until the Certification Period End date indicated in Section 2 - Form Information has passed. The optimal time to submit is on the due date. If the due date has already passed, please submit this form as soon as possible. It is important to note that you may receive multiple requests for certification throughout your leave.

Section 3 - Disability Status

During your leave, you should indicate that you were unable to perform your regular work due to your disability for the entire certification period covered by this request.

Section 4 - Money, Wages, or Workers’ Compensation

If you are receiving compensation while on leave, please select "Yes."

Section 5A - Pay Information

  • Enter the employer's legal name in the field labeled "Paid by."

  • Specify the "Paid from" and "through" dates.

  • Indicate the "gross dollar amount received."

From the available options, please select "Other" as the payment type for top-up pay, and then enter "LSDI - Integrated Wages" in the designated box.

Section 5B - Payment Information

This screen displays the information you provided in Section 5A and offers the option to add additional employers or wages from another source.

Section 6 - Certification

Please include the relevant Cocoon Pay Program Explainer statement for online submission in the “Additional Information” box to clarify the type of pay being received while on leave, keeping in mind the character count limitation.
